This position is funded by a grant from HRSA to provide Behavioral Health Services to the underserved populations (homeless and housed) of Westchester County receiving care at the MVNHC Network.
Essential Duties
- Conducts street outreach within our catchment area to identify homeless individuals/families
- Performs HIV counseling and testing to high risk individuals
- Observes HIV Confidentiality Law and abide by HIPAA
- Be knowledgeable and informed on a variety of community resources
- Work closely with diverse populations and/or at-risk communities
- Work closely with multidisciplinary team of providers
- Provide referral information to homeless individuals about MVNHC network services
- Assist in the delivery of health services upon registration
- Attend relevant job training when scheduled
- Perform other related duties as assigned or requested by supervisor
Qualifications
- Graduation from an accredited high school or possession of a high-school equivalency certificate. Associates Degree is a plus
- 5+ years of experience with homeless individuals and/or Community Health
- Knowledge of community resources, outreach methodology, and HIPAA
- Computer literacy required
- Must be flexible and possess good interpersonal skills
- Ability to work with a multidisciplinary team of providers and a willingness to seek and accept supervision, as needed
- Be able to work collaboratively with other CBOs and within a multidisciplinary setting
- NYS valid driver’s license
